Alexander’s Supported Living works in partnership with local authorities, NHS services, families and individuals to provide structured supported living arrangements that promote stability and long-term independence. We understand that referrals often take place during periods of change or uncertainty, and our priority is to make the process clear, collaborative and professionally reassuring. From initial enquiry through to assessment and service delivery, we aim to provide consistent communication and realistic guidance at every stage. Our experienced teams are accustomed to supporting complex mental health needs and carefully managing transitions into community living. By working closely with commissioning teams and clinical professionals, we help ensure placements are appropriate, sustainable and aligned with agreed care outcomes.

Who Can Make a Referral

Referrals to Alexander’s Supported Living are typically made by professionals coordinating care and accommodation arrangements, although individuals and families may also contact us for guidance. We are happy to discuss potential suitability at an early stage to help ensure the right pathway is identified.

Local Authorities & Commissioners

We regularly work with social workers, brokerage teams and commissioning managers to support individuals who require structured community-based care following assessment or review.

NHS & Clinical Professionals

Community mental health teams, discharge coordinators and clinical practitioners may refer individuals who would benefit from supported living as part of a longer-term recovery or stability plan.

Families & Individuals

Family members or individuals themselves may contact us to explore supported living options. We can provide guidance on eligibility, referral routes and how services may be introduced safely.

Assessment and Placement

Following an initial enquiry, we carry out a structured assessment process to understand mental health needs, risks, daily living skills and longer-term aspirations. This collaborative approach ensures that any proposed placement is appropriate, proportionate and aligned with wider clinical or social care plans.
